Adult Community Support Services
Community Support Services (CSS/CSS-At Risk)
CSS offers a variety of supports to adults over the age of 18, living in Burlington County, and diagnosed with a serious mental illness to increase independence and live successfully in the community. Community Support Services offers mental health rehabilitation and support services that assist individuals with the skills necessary to achieve their wellness and recovery goals. Services include skill building, education/employment assistance, service linkages, advocacy, peer support services, and housing assistance.
Projects for Assistance in Transition from Homelessness (PATH)
This program provides community-based case management and outreach services to Burlington County adults who are dealing with a serious and persistent mental illness and are either homeless or at risk of becoming homeless. This program also provides targeted case management services for military veterans experiencing homelessness and mental health issues. Individuals eligible for PATH must meet HUD’s (Department of Housing and Urban Development) definition of homelessness, experience chronic homelessness, be able to provide a certification of homelessness from a community provider that provides homeless services, or are at imminent risk of homelessness (within 14 days).
Enhanced Community Support Services (ECSS)
The ECSS program is for adults being released from state psychiatric hospitals. ECSS offers a higher level of support and case management services for adults living in Burlington County with 24-hour on-call support.
DDMI (Developmentally Disabled Mental Illness) Enhanced Community Support Services (DDMI-ECSS)
This program is for adults being released from state psychiatric hospitals with a mental health diagnosis and are DD-like. The program offers a higher level of support and case management services for adults living in Burlington, Mercer and Bergen County.
Please note that we do not have our own housing, as we assist individuals with obtaining affordable housing.
